Make OHIF less meteor-dependent

• support custom viewers without having to use meteor
• support various combinations of client and server with or without meteor
• best "value" by using OHIF with meteor, but not required
• refactor ViewerBase (move from DDP to REST)
• don't depend on deploying MongoDB and Meteor server
• MiniViewer = OHIF Viewer + orthanc

OHIF integration with Slicer

• expose 3D slicer's functionality via OHIF Viewr
  • MPR/slab, volume rendering, segmentation
  • Hanging protocol support
  • use REST APIs and server-side rendering
• integration prototype
  • lung lesion segmentation in a web browser
